Conviva Care Centers Teams with Strive Health to Provide Kidney Disease Care Management to Patients in South Florida

MIAMI & DENVER , September 28 /Businesswire/ - Conviva Care Centers, a network of 300 primary care physicians that specializes in treating seniors, and specialized kidney care company Strive Health today announced they are partnering to offer specialized kidney disease care management services for eligible patients in South Florida.

Strive Health and Conviva Care Centers will integrate technology and clinical infrastructure to improve health outcomes for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Strive Health’s care teams will work with Conviva’s primary care physicians, providing care expertise for late-stage CKD and ESRD patients.
