Conviva Care Centers Teams with Strive Health to Provide Kidney Disease Care Management to Patients in South Florida

MIAMI & DENVER , September 28 /Businesswire/ - Conviva Care Centers, a network of 300 primary care physicians that specializes in treating seniors, and specialized kidney care company Strive Health today announced they are partnering to offer specialized kidney disease care management services for eligible patients in South Florida.

Strive Health and Conviva Care Centers will integrate technology and clinical infrastructure to improve health outcomes for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Strive Health’s care teams will work with Conviva’s primary care physicians, providing care expertise for late-stage CKD and ESRD patients.

“Partnering with Strive Health will enable us to provide additional critical health services to our patients,” said President of Conviva Physicians Group Steve Lee, MD. “At Conviva, we measure our success by the long-term health of our patients, and we believe working together with Strive Health’s clinicians will help us advance the preventive, personalized health care we offer our patients.”

Together, Strive Health’s clinical teams and Conviva’s primary care physicians will provide care management services to patients, aiming to reduce hospitalizations and dialysis crash rates, increase home dialysis utilization, detect kidney disease earlier and connect patients to nephrologists and other specialists where and when it’s most important.

“Strive delivers comprehensive and compassionate kidney care,” said Chris Riopelle, Chief Executive Officer of Strive Health. “By combining Strive’s specialized care model with Conviva’s innovative primary care physicians, we can develop a comprehensive and integrated system that transforms patient health outcomes and experience.”

As CKD is the nation’s ninth-leading cause of death, a proactive approach to kidney care is critical in patient health and well-being. Conviva’s partnership with Strive represents a proactive industry shift toward preventative and holistic care.
